Please note: Your application will not be processed until $75 fee is received. For your convenience, the teacher recomendation form and grade request form are provided below. Please download and submit to your elementary school.
If you are mailing check or money order, please write the students name and "application fee" in the memo. Mail payments to:

Testing Information
If you chose St. Elizabeth as your test location, you will automatically be registered for the test on Sat. Jan 10 8:30-12. Please arrive at 8:15 a.m.
